I teach documentary theory and practice and these are the books I recommend
to my students:
Directing the Documentary by Michael Rabiger, Focal Press, 2004.
Documentary Storytelling for Video and Filmmakers by Sheila Curran Bernard,
Focal Press, 2004.
Writing, Directing, and Producing Documentary Films and Videos by Alan
Rosenthal, Southern Illinois University Press, 2002.
Cross-Cultural Filmmaking: A Handbook for Making Documentary and
Ethnographic Films and Videos by Ilisa Barbash and Lucien Taylor, University
of California Press, 1997.
Introduction to Documentary Production: a Guide for Media Students, edited
by Searle Kochberg, Wallflower Press, 2002.
I also find "Cross-Cultural Filmmaking" very useful.

I second that recommendation. "Cross-Cultural
Filmmaking," is a very useful and comprehensive text.
