You need two licenses:
1. Sync - with the publisher of the song (identify the publisher by doing a
search on BMI or ASCAP, then contact them with that contact information.
http://www.bmi.com/
http://www.ascap.com/index.html

BMI had a page on doing licensing as well. You should come up with an idea
of what rights you want before contacting the publisher.

2. Master - for use of the actual recording, usually held by the recording
company

You DO NOT need a BMI or ASCAP license those are for restaurants, radio,
etc.
Just start with a title search on BMI & ASCAP. Ideally the CD from which
you took the somg will indicate with which service the publishing is listed.
Should also list the publisher. And the label that made the CD will
probably be the owner of the Master rights, unless otherwise indicated.

Iım sure there are various other web pages that lay it out as well.

Best,

Adam
