I've used DJ Audio (not DJ Sound) for tons of restoration track negatives and my own work, and they're great. I know a lot of L.A. independent filmmakers like NT Audio as well, including a few who are particularly meticulous and exacting.

Chris Millar (Noise & Light) is no longer shooting optical tracks. 40 Frames has the old Noise & Light equipment. We are preparing to to offer this service in 2011.
In the meantime, try Gibbs Chapman (Non Productions) in San Francisco, or DJ Sound or NT Audio in LA.
